Our goal is to ensure that all students have access to quality education, regardless of their medical conditions. We understand that some students may face health challenges that prevent them from attending school in a traditional setting. The Hospital/Homebound Services Department is here to provide educational continuity and support during these times.
Students of the Bogalusa City Parish School District who are unable to attend school because of physical disability may be assigned a properly certified teacher who will instruct them at home or in a hospital. In order for a student to be considered for Hospital/Homebound (H/H) instruction, the student, as a result of health care treatment, physical illness, accident, or the treatment thereof, must be temporarily unable to attend school for the provision of regular educational services. Such incapacitation shall require verification on the application by a physician. The H/H instruction program is a continuation of the regular school instructional program, designed to help the child return to school without falling too far behind in assigned work. The student shall be under the same state and local regulations as any other student. A responsible adult must be present where instruction is provided.
Eligibility:
This program is available for students currently enrolled in the Bogalusa City School District who have a medical condition that necessitates absence from school for an extended period (10 days or more).
A licensed physician must provide documentation verifying the student’s condition. Physician must be a specialist in the area for which the student is being placed on hospital/homebound, i.e. orthopedist, psychiatrist, obstetrician, etc.
Grade Levels: We serve students from Kindergarten through 12th grade.
According to the Louisiana Department of Education guidelines, “homebound instruction should be used as a last resort after all other options and/or alternative schedules i.e., half days, work packets) have been exhausted.”
Instruction: Certified teachers provide instruction in core academic subjects. Depending on the student’s needs, instruction may be delivered in person, at home, in a hospital setting, or through virtual platforms.
Individualized Education Plan (IEP) Support: For students with IEPs, our services work in collaboration with special education teams to ensure goals are met.
Academic Coordination: We coordinate with the student’s regular teachers to ensure continuity in curriculum and assignments.
Referral: Contact the school counselor or administrator to initiate a referral for Hospital/Homebound Services.
Medical Documentation: Submit the required application (HH2 or HH3) and medical documentation from a licensed physician. *see forms below
Review and Approval: The application will be reviewed, and if approved, services will be initiated promptly.

This application must be faxed or mailed directly to the Hospital/Homebound office fax number or address at the bottom of this page from the physician’s office, along with the HH2B form completed by the physician. The medical condition must be significant enough that it impairs the student’s ability to function in school and job/home related activities.
